In order to be considered for the Prize, regardless of the category, applicants should submit the following:

1. A completed mid-career scientist application form and senior scientist application form (mandatory fields must be filled out to be eligible for the Prize).
2. A Curriculum Vitae (CV).
   Please use the following format for your CV:
   Country-category-CV-ApplicantName.
   Example: Indonesia-Senior-CV-JaneDoe
3. A letter of recommendation from an academic or professional contact.
   Please use the following format for your letter of recommendation:
   Country-category-REC1-ApplicantName.
   Example: Indonesia-Senior-REC1-JaneDoe
4. A letter of support from a female mentee or other women who have been impacted by the applicant.
   Please use the following format for your letter of recommendation:
   Country-category-REC2-ApplicantName.
   Example: Indonesia-Senior-REC2-JaneDoe

All documents are to be submitted online at scienceprize4women.asean.org. the application form can be downloaded from that website. There is no preferred form for the CV.

Please limit of your uploaded documents, not to exceed 2 Mb.

Please send all files in pdf format. You may add separate/individual pdf files when uploading, to be sent one time (all files at once).

You are only allowed one time to upload all documents at the same time to send your application.

All four documents (application form, CV and two letter of recommendations) must be received by the Prize administrators before the application submission deadline for an application to be considered.
